3 Telebet Identity Card
| 3.1 | A TIC will be issued to each person opening an account; |
| 3.2 | A Holder is required to sign his TIC in the space provided immediately on receipt thereof. |
| 3.3 | A TIC will carry the name, signature and account number of the holder. |
| 3.4 | The holder's pincode will not appear on a TIC. |
| 3.5 | In the event that a TIC is lost by the holder, it shall be the duty of the holder to notify TAB of such loss by following the procedure set out in 1.1.1, 1.1.2 and 1.1.3 of the Telebet Operational Procedures, on compliance of which, TAB will impose an administrative penalty and issue to the holder a new TIC. TAB will not be responsible for any loss or damage suffered by the holder as a result of the loss of a TIC. |
| 3.6 | No person may hold more than one TIC for any one account at any time, without the prior written approval of TAB. |
